COVID-19 Vaccine Booster Shot (Third Dose)

Overview

【Eligible persons】Nonoichi residents who are aged 18 or above, and have had their second dose at least 8 months ago (in principle)

【Vaccination method】Medical Institution Vaccination, Collective Vaccination

【Number of shots】One

【Type of Vaccine】Pfizer, Takeda/Moderna

Procedure

1. A vaccine ticket will be posted to you in an envelope

2. Make a reservation

Reservation method differs depending on whether you get vaccinated at a medical institution or at a collective vaccination venue.

3. Get vaccinated at your reserved time and location

Please bring all the documents in the envelope and a proof of identity.

・Fill out and submit Prevaccination Screening Questionnaire(予診票)

・Get screened and vaccinated

・Vaccination Certificate will be issued

・Wait and observe for any adverse reactions (15~30 minutes)

Vaccine Ticket Posting Schedule

Booster shot vaccine ticket will be posted based on the date of your second dose. As such, members of the same residence or age group may receive the ticket on different dates.
